Student Registration

Instructions for registering your child can be found below:

New Students - Applicable Forms can be downloaded below.

  1. Birth Certificate
  2. Four (4) proofs of residency. One (1) proof must either be the most recent tax bill or lease agreement.  Other proofs can be any mail that has the correct name and address.
  3. Custody papers, if applicable

Current Students -    Applicable Forms can be downloaded below.

  1. Two (2) proofs of residency provided no changes in residency have occurred within the last year

Please also bring phone numbers for your child's doctor and dentist as well as phone numbers of all emergency contacts.

Parents are also reminded physicals are required for students entering Grades Kindergarten & 6th.  Dental exams are required for students entering Grades Kindergarten, 2nd, and 6th.  Vision exams are required for all students entering Kindergarten.  More information can be found HERE.

New Student Registration Checklist

Kindergarten Registration

Kindergarten registration for the upcoming 2023-2024 school year is upcoming.  Initial registration will be online.  You will click a link to begin the process.  Once this is done, the district will approve you so you can complete the paperwork portion of registration.  After this is completed, parents will need to come to the district office at 408 South Belleville Street to provide the following: 

  1. Birth Certificate
  2. Lease Agreement (if renting) or most recent tax bill (if you own).
  3. Three (3) other pieces of mail with name and address.
  4. Custody Papers, if applicable.
In addition, parents/guardians will need to fill out the forms for NEW students found above.  Feel free to complete the forms now and bring them to registration.  For questions, please call 539-3188.
