Asthma, Seizure and Food Allergies

If your child has a food allergy, is susceptible to seizure, or has a food allergy, please complete the appropriate Action Plan form below and submit to Ms. Lauren Baker or Ms. Whitney Schoenbeck.

Physical Examination

All students entering Pre-School/Early Childhood, Kindergarten, 6th grade, or entering an Illinois school for the first time need to have a new physical exam and all required immunizations completed by August 1 each year.  You can access the form HERE

Tdap Vaccine Requirement

All students entering, advancing or transferring into grades 6-8 will be required to show proof of receipt of one dose of Tdap vaccine.

Dental Examination

All students entering Kindergarten, 2nd grade, or 6th grade or entering an Illinois school for the first time must have a dental exam completed.  

Vision Examination

All students entering Kindergarten or entering an Illinois school for the first time need to have a vision examination before August 1 of their entrance year to school.  You can access the form below.

Sports Physical

A Sports Physical Form is required to be completed each year for those students in grades 5th, 7th and 8th who want to try out for a sport.  All 6th graders are required to have the State of Illinois Certificate of Child Health Examination Form completed in lieu of the Sports Physical Form.  All students trying out must have a current physical on file BEFORE their first practice and a signed Concussion Form.
